Just a few reminders
You will receive an email that invites you to join a platform called Practice Better. You must create an account there, or we can’t meet for our virtual appointment!
Practice Better will be the “hub” of our relationship. We can exchange documents, messages, my notes, and other information here.
Our appointments will also be within Practice Better!
At the time of your appointment, simply log back in, and I will connect us. Super easy.
Paperwork
In addition to the welcome email, you will receive some forms to fill out. All of this will be done within Practice Better as well.
This paperwork must be completed within 24 hours of our appointment. Otherwise, we will have to cancel your appointment and reschedule for a later date.
Give yourself a bit of time to complete, as the new client questionnaire is a bit of a doozy!
In addition to the forms, please upload your insurance card (front and back) and your driver’s license (front) in the documents section.
Insurance
I accept BlueCross BlueShield, United Healthcare, and Aetna insurance plans. I do not have contracts with Medicare, TriCare, or Medicaid.
It is your responsibility to understand your insurance benefits. Most plans cover nutrition at 100% for preventative counseling. Starting January 2021, some BlueCross plans may charge a small co-pay for virtual sessions.
If you want to be sure, call the number on the back of your Insurance ID card. They’ll be able to tell you how many sessions will be covered and if any services will go towards your deductible. If they ask for a CPT code, get information for both 97802 and 97803.
Also, keep in mind that any quotes of coverage given to either you or me before the date services are provided are estimated only.
Miscellaneous
- Go You! Nutrition Counseling has a no-show/late cancellation policy. Any appointment that is canceled without 48 hours notice will incur a $50 fee that must be paid before rescheduling the appointment. We understand that extenuating circumstances exist. This fee may be waived on a case-by-case basis.
- Medical Nutrition Therapy, or nutrition counseling, by a dietitian is not a one-size-fits-all approach. Nor is it a list of rules followed by a meal plan. Instead, we aim to create personalized nutrition “patterns” that allow you to enjoy your favorite foods while creating behavior changes the lead to permanent lifestyle modification rather than quick-fix diets that you drop once you meet your goals or get irritated with the plan.
- To ensure your privacy is protected as possible, please try to communicate via the Practice Better messaging system, or call me at 214-621-0584. Email (jennifer@goyounutrition.com) is also possible, but just the slightest bit less secure. We strive to follow HIPAA guidelines in the strictest way possible.
- Your first meeting with the dietitian is ~90 minutes long. (Those with Aetna will be restricted to 1 hour). Keep this in mind when you schedule for the first time! Appointments will get shorter as we continue our relationship.
- Sometimes questions pop up between sessions. I encourage you to write these down so you don’t forget them before our next appointment. Feel free to shoot me a message in Practice Better. If it’s something I can answer very quickly, I’ll respond when I’m able. However, longer, more detailed questions will be deferred to the next appointment.
- Any lab work you have had would be super helpful in determining your next steps. If you’ve had lab work done, please upload them to your documents section of Practice Better or have your doctor fax them to me at 844-782-7109.
